We are searching for a talented LPN Telehealth-VNA.
Hours: 40 hrs
Shift: Day shift, 8:00am - 4:30pm with weekend and holiday rotation
Location: Visiting Nurse Association - Fairhaven, MA

ResponsibilitiesUnder general supervision of the TL-Telehealth Program or designee, perform duties to support the Telemonitoring department to reduce re-hospitalizations and provide proactive symptom management while adhering to best practices. Address data packets sent via the telehealth system with patients, field staff, physicians, and clinic staff. Triage phone calls and provide patient education as it relates to data collected via telephone as needed.
Troubleshoot equipment via telephone with occasional need for home visits to patients for equipment set up or problems. Provide video visits in compliance with patient care plan.
- Graduate of an NLN accredited school of Nursing (LPN)
- Current LPN licensure in both RI and Massachusetts required
- Basic Life Support (BLS) and CPR certification required
- Proficient computer skills, including experience with various software, database and electronic records programs
- Two (2) years of related work experience required; recent home care experience preferred
- Recent Telehealth and/or Cardio Pulmonary experience preferred
- Must be fully vaccinated against seasonal influenza and COVID-19 or be exempt with a signed statement
Pay rate will be determined based on level of experience.
Pay RangeUSD $27.29 - $41.87 /Hr.
