Post-Acute Cardiology Care, LLC (PACC) is an independent medical practice dedicated to providing specialized cardiovascular care to high-risk patients in skilled nursing facilities (SNFs). Founded in 2014, PACC addresses the growing needs of older adults with cardiovascular conditions transitioning to SNFs after hospitalizations. The practice aims to reduce hospital readmissions and assist in successful patient discharge to their communities. PACC takes a compassionate, evidence-based approach that integrates geriatric principles and personalized care for patients with complex conditions, multimorbidity, and frailty.

Post-Acute Cardiology Care, LLC is seeking a skilled and compassionate licensed nurse practitioner interested in joining a patient-centered skilled nursing facility-based cardiology consulting practice.

• Round in designated skilled nursing facilities
• Complete structured clinical interview with all new patients
• Order and study diagnostic tests (blood tests, X-rays etc.)
• Propose treatments for acute and chronic diseases
• Document all patient care in electronic medical record
• Educate patients and their family members on diseases, epidemics and prevention

• Valid Connecticut RN license
• Comprehensive knowledge of adult nursing care
• Comprehensive knowledge of principles of nursing, and physical assessment skills
• Ability to function autonomously, excellent leadership, clinical & interpersonal skills
• Strong organizational and administrative skills
• Strong ethics
• Master’s degree;
  PhD is a plus
• 20-30 hrs/week

Flexible pay schedule, range depends on hours worked/productivity. Option for independent contract vs salary
