From day one, you will play a central role in shaping how care is delivered in a community where access truly matters. • Be a founding clinician with real ownership in how the clinic operates and grows
• Practice at the top of your license with time to build meaningful patient relationships
• Help redefine rural healthcare through integrated, team based care
• Use telehealth and digital tools to expand access without sacrificing quality

• Deliver comprehensive primary care with a strong focus on prevention and continuity
• Serve as a clinical leader and trusted partner to patients and families
• Collaborate with pharmacists, care coordinators, and virtual care partners
• Help design workflows, care standards, and patient experience from the ground up
• Contribute ideas and leadership as the clinic scales and evolves

• A Nurse Practitioner with a Master’s or Doctoral degree in Nursing
• Licensed in Pennsylvania with national certification such as FNP-BC or FNP-C
• Experienced in primary care, community health, or rural health environments
• Confident using EHR systems and telehealth platforms
