What You Will Do
  • Deliver comprehensive primary care with a strong focus on prevention and continuity
  • Serve as a clinical leader and trusted partner to patients and families
  • Collaborate with pharmacists, care coordinators, and virtual care partners
  • Help design workflows, care standards, and patient experience from the ground up
  • Contribute ideas and leadership as the clinic scales and evolves
Who You Are
  • A Nurse Practitioner with a Master’s or Doctoral degree in Nursing
  • Licensed in Pennsylvania with national certification such as FNP-BC or FNP-C
  • Experienced in primary care, community health, or rural health environments
  • Confident using EHR systems and telehealth platforms

    Entrepreneurial, adaptable, and excited by building something meaningful
  • Deeply committed to patient centered, relationship driven care
