Job Description

Role:

The Community and Family Services (CAFS) case worker will promote the values of CORE Community Services to provide clients with greater access and opportunities to develop independence, increase participation, gain access to quality care and achieve self-fulfilment.

The CAFS case worker will deliver culturally safe, responsive and flexible services to meet the needs of young people, families and communities, who are at risk or experiencing vulnerability within the Fairfield,Liverpool and Campbelltown local Government areas. The Program will target early intervention and prevention models through evidence-based workshops, group activities and community development including events with the aim to promote relevant internal services.

The CAFS case worker will collaborate with various stakeholders in the community such as local high schools, educational and employment programs and other government and non-government organisations. The program will ensure and enhance sensitivity and knowledge of the issues and needs of young people, families and communities including First nations and culturally and linguistically diverse people are being met.

In addition to this, the CAFS case worker will also provide individualised support to young people aged 12 to 25 years, who reside within the South Western Sydney region. This could be implemented through group work or one on one case management support for a young person that enables independence and autonomy and prioritises the young person’s voice in decision making.

Young people will be provided with strengths based, flexible, trauma informed and client centred case management while practically supported to build their capacity and self-determination. Case workers will connect young people to various internal, community and specialist support services to develop greater independence, self-efficacy and long-term outcomes.

Responsibilities
  • Undertake and meet all case management responsibilities in relation to young people allocated by the Community and Family Services Manager with a manageable caseload
  • Maintain regular, effective and meaningful contact with young people.
  • Work within a client-centred, trauma-informed framework that is culturally appropriate.
  • Ensure that case plans are regularly reviewed and facilitate the attendance of all key stakeholders at relevant meetings including case conferences and reviews.
  • Develop, implement and evaluate Centre based programs in order to address the issues and interests of young people, families and community
  • Collaborate with internal and external agencies to facilitate early intervention and prevention workshops with aims to increase community awareness of local support services.
  • Work within a client-centred, trauma-informed framework that is culturally appropriate.
  • Facilitate evidence-based workshops in schools that centre around personal development and capacity building activities that are in line with the Community and Family Services Outcomes Framework
  • Coordinate and implement community events, activities/excursions and festivals to promote awareness of services offered by CORE CS
  • Provide clear and concise advice, assistance and/ or recommendation strategies to Young People and their families in South West Sydney region
  • Actively marketing and promoting CORE Youth Services programs and activities to new prospective participants and service providers.
  • Complete appropriate referrals for young people, families and persons in community to relevant services to adhere to the No Wrong Door policy
  • Develop and maintain positive and effective professional working relationship with internal and external stakeholders.
  • Actively participate in local and sector networks, working groups and community interagency groups
  • Ensure that all appropriate data is submitted within the set timeframe and are always kept confidential and in a locked cabinet
  • Work collaboratively with staff to facilitate group work activities in line with the Community and Family Services (CAFS) Outcomes Framework.
  • Complete and submit risk assessments, program proposals and evaluations to the manager in accordance with the departmental contractual agreement within set timeframe
