As home health nurses, you have an opportunity to teach and educate your patients, apply your skills for treatments, and carry doctor’s orders based on individual patient s needs.

We are currently seeking dedicated healthcare professionals to join our team. As a member of Mare Island Home Health, you will have the opportunity to make a profound difference in the lives of our patients and veterans by providing comprehensive care tailored to their unique needs.

• Conduct thorough assessments and develop individualized care plans.
• Provide skilled nursing care, therapy services, or social work support according to prescribed treatment plans.
• Educate patients and caregivers on disease prevention and management.
• Collaborate with interdisciplinary team members to ensure holistic patient care.
• Document all patient care services accurately and timely.
• Flexible time for home visit (per diem role).
• One on one with your patient.
• Independent use of professional skills.
• Continues education and professional growth.

• Valid California state licensure or certification in your respective field (RN, LVN, PT, PTA, OT, COTA, ST, MSW).
• Current CPR certification.
• Current CA driver s license plus proof of car insurance.
• Minimum one year OT clinical experience, (two+ years is more preferable).
• Computer/Internet experience (online charting to be used).
• Bi/Multi-lingual skills a plus (e.g. Spanish, Russian, Tagalog, etc.).

• Ability to travel to patient homes across Solano County, Napa County, and Contra Costa County.
