At North Bay Health the LCSW provides direct clinical social work services, including counseling, discharge planning, and case management for hospital patients and their families. Responsibilities include performing biopsychosocial assessments, documenting emotional, psychological, financial, and environmental needs, initiating and discontinuing 5150 custodial holds within North Bay Medical Center and Vaca Valley Emergency Rooms, and collaborating with interdisciplinary healthcare staff. The LCSW manages the psychosocially complex patient across the continuum of care by developing a comprehensive care plan, monitoring progress toward goals, and identifying barriers to timely discharge.
The role also facilitates integration with community-based resources to optimize patient health and communicates appropriate information to care providers in the continuum of care. All activities are consistent with North Bay Health’s mission, values, and applicable regulations and policies.
- Master of Social Work from a Council on Social Work Education accredited school.
- California BBS Licensed Clinical Social Worker (LCSW). CCS paneling required for work with the pediatric/Neonatal Intensive Care (NICU) population.
- Minimum of two years’ experience within an acute health care setting or a community health setting.
- Strong clinical assessment and critical‑thinking skills with independent judgment and clinical decision‑making.
- Excellent interpersonal, written, and listening skills; ability to convey instructions and information accurately.
- Effective follow‑through, organizational, and time‑management capabilities.
- Ability to direct actions of others to accomplish tasks and functions.
- Competence with diverse populations, including age, culture, ethnicity, and gender considerations.
- Knowledge of child, elder, and dependent adult abuse, domestic violence reporting, and related legal requirements (Tarasoff, HIPAA, etc.).
- Understanding of suicide risk assessment and treatment planning; proficiency with DSM‑V and 5150 custodial hold procedures.
- Basic proficiency with word processing; experience with Cerner and PowerPoint preferred.
- Knowledge of acute, subacute, long‑term, home, hospice, rehabilitation, and community resource options preferred.
- Effective caregiving and advocacy for diverse clients.
- Ability to build and maintain constructive relationships with team members, external providers, and system managers.

- Adheres to established policies, procedures, and attendance/punctuality requirements.
- Essential functions include standing, walking, stooping, bending, twisting, intermittent sitting, and computer use.
- Transportation provided for referrals when needed.

- Based on business need: typical schedules 7:45 am‑4:15 pm or 8:00 am‑4:30 pm, Monday‑Friday. Weekend and holiday coverage may be required.
- Hourly salary range: $53.29 – $64.77 (based on years of experience).
