We are currently seeking a compassionate and dedicated Community Health Worker (CHW) to join our team. The CHW is primarily responsible for sourcing, establishing, and maintaining relationships with client referral sources and prospective clients. The position is responsible for providing outreach and education regarding the Agency’s services to potential clients and referral sources and bringing new clients into the Agency. In addition, the position collects market and competitive data and reports such information to management along with recommendations for potential solutions.
The responsibilities include driving client growth and developing new referral relationships, developing new referral sources, enhancing existing referral relationships, preparing and delivering sales presentations, meeting with prospective clients, closing sales, assisting Client Relationship Managers, communicating with and supporting existing clients, monitoring competition, recommending changes, resolving client complaints, maintaining CRM systems, forecasting sales activity, developing innovative sales strategies, meeting personal sales targets, attending meetings, traveling to defined geographies, and following company policy and regulations.
- Responsible for driving client growth and developing new referral relationships
- Develop new referral sources by planning and organizing schedule to call on potential referral outlets and other sources of prospective clients
- Enhance existing referral relationships by presenting current service offerings, developing new solutions, and communicating the agency’s capabilities and vision
- Prepare and deliver sales presentations and tailor each presentation to the particular audience
- Meet with prospective clients in their homes to discuss agency’s services, payment options, and onboarding process
- Close the sale with individual prospects using various sales methods to ensure prospects select and remain with Personal Home Care
- Assist the agency’s Client Relationship Managers in processing and onboarding new clients
- Communicate with and support existing clients as needed in conjunction with PHC’s Client Relationship Managers
- Monitor competition by gathering current marketplace information on pricing, services, new entries to the market, pay rates, marketing techniques, etc
- Recommend changes in services, pricing, and policy by evaluating results and competitive developments
- Resolve client complaints by investigating problems, collaborating with internal resources, developing solutions, preparing reports, and taking action or making recommendations to management
- Keep management informed by updating CRM systems, submitting sales activity and results reports, such as daily call reports, weekly work plans, and monthly and annual territory analyses
- Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks and participating in professional societies
- Forecast sales activity for budgeting purposes
- Develop “out of the box” sales strategies and models and evaluate their effectiveness
- Meet personal sales targets
- Attend meetings, sales events, and trainings to keep abreast of agency’s latest developments
- Travel to defined geographies to sell agency’s services
- Follow company policy and adhere to state and federal regulations
- Bachelor’s Degree or equivalent
- Reliable transportation and a valid driver’s license
- Ability to pass background checks as required by state regulations
- Proven sales experience with track record of over‑achieving quota
- Demonstrated teamwork capabilities with the ability to relate well to staff and other professionals
- Experience with office productivity software (Microsoft Office) and CRM systems
- Familiarity with different sales techniques and pipeline management
- Strong communication, negotiation, and interpersonal skills
- Self‑motivated and driven
