We are currently seeking experienced and compassionate Registered Nurses (RNs) to join our home health care team. The Registered Nurse will be responsible for providing skilled nursing care to our clients in their homes, coordinating care plans, and collaborating with other members of the healthcare team. Responsibilities- Assessment and Care Planning:
Conduct comprehensive assessments of clients' health status, including physical, emotional, and psychosocial factors, to develop individualized care plans. - Skilled Nursing Care:
Provide skilled nursing care and interventions, including wound care, medication management, IV therapy, catheter care, and other nursing procedures, as prescribed by the physician. - Patient
Education:
Educate clients and their families on disease management, treatment plans, medication administration, and self-care techniques to promote independence and improve health outcomes. - Care Coordination:
Coordinate and collaborate with other members of the healthcare team, including therapists, social workers, and home health aides, to ensure seamless continuity of care and optimal client outcomes. - Documentation:
Maintain accurate and up-to-date documentation of nursing assessments, interventions, and client progress in electronic health records (EHR) or other documentation systems, in compliance with agency policies and regulatory requirements. - Medication Management:
Administer medications, monitor medication effectiveness and side effects, and ensure adherence to medication schedules, as prescribed by the physician. - Patient Advocacy:
Serve as an advocate for clients and their families, ensuring their needs and preferences are communicated and addressed effectively by the healthcare team. - Safety and Infection Control:
Implement infection control measures, assess safety hazards in the home environment, and educate clients and caregivers on injury prevention and safety practices.
- Current unrestricted state licensure as a Registered Nurse (RN).
- Minimum of two (2) years of clinical experience as an RN, preferably in home health care or a related setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Compassionate, patient-centered approach to care.
- Ability to work independently and manage a diverse caseload effectively.
- Ability to handle physical demands of the job, including lifting, bending, and assisting with transfers.
- Active CPR Certification
- Reliable transportation and a valid driver's license with active automobile insurance.
- Ability to pass background checks as required by state regulations.
