Licensed Practical Nurse (LPN) – Home Centris Home Health
We are looking for experienced LPNs to provide quality care and nursing oversight of our Medicare skilled home‑care services in clients’ homes. Your daily routine will involve starting and visiting clients, ensuring care plans are followed, and collaborating with the Home Health team.
Key Responsibilities- Provide skilled nursing care in clients’ homes.
- Start and conduct routine visits, following care plans.
- Collaborate with the Home Health team to monitor patient status.
- Work independently, making clinical decisions appropriate to the LPN scope.
- Current and unencumbered LPN license in the State of Maryland.
- 2-3 years of LPN experience; home‑care experience strongly preferred.
- Ability to work independently and make clinical decisions.
- Clean driving record; minimum one year Maryland driver’s license.
- Positive attitude and commitment to client care.
- Monday‑Friday availability; typically 3‑4 patients per day. Some weekend starts may be required.
Pay $45.00 per visit, not per hour.
PRN – flexible shifts, Monday through Friday, with occasional weekend starts.
