Position: Developmental Disability Specialist III - Support Coordination

Job Announcement

This position supervises a team of Support Coordinators and provides clinical direction to them in areas including clinical assessment of client needs (including treatment modalities, intervention techniques, behavior management, independent living skills, supportive counseling, family education, crisis intervention, housing, communication, recreation, financial, medical, transportation, vocational, legal, education, etc.);
Interdisciplinary Team Leadership; formulating, writing and developing Person Centered Plans; continuous individual progress review and evaluation, use of assessment tools; referral methods; case consultation, documentation, and special projects. Assists DDSII Support Coordinators in determining clinical interventions and in problem solving in order to enhance their knowledge, abilities, and clinical skills. Supervises Support Coordinators administrative duties. Provides training and education regarding inter and intra-agency services and clinical interventions.

Provides oversight and review of submissions required to maintain services under the DD Waivers (Service Modifications, Renewals, etc.) including submissions in WaMS (Waiver Management System). Participates in record and utilization reviews. Assumes caseload duties in the absence of supervises. Provides Crisis Intervention Assistance. Participates in Emergency 24-Hour on-call rotation for service provision. Oversees the use and maintenance of the electronic health record to record all aspects of Support Coordination performance as well as to document the delivery of services.

    Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Clinical Psychologist, Licensed Marriage and Family Therapist, Licensed Substance Abuse Treatment Practitioner or Clinical Nurse Specialist.
  • License/Certification Reimbursement - Reimbursement for job related licenses and/or certifications once per year.
