Overview

This position supervises a team of Support Coordinators and provides clinical direction in areas including clinical assessment of client needs, formulation and development of Person Centered Plans, continuous progress review, use of assessment tools, referral methods, case consultation, documentation, and special projects.
Assists DDSII Support Coordinators in determining clinical interventions and in problem solving to enhance knowledge, abilities, and clinical skills. Supervises administrative duties, provides training and education regarding inter- and intra-agency services and clinical interventions. Provides oversight of submissions to maintain services under the DD Waivers (Service Modifications, Renewals, etc.) including submissions in WaMS. Participates in record and utilization reviews. Assumes caseload duties in the absence of supervises.
Provides Crisis Intervention and participates in Emergency 24-Hour on-call rotation. Oversees the use and maintenance of the electronic health record to document all aspects of Support Coordination performance and service delivery.

Responsibilities- Provide clinical direction and supervision to Support Coordinators; conduct clinical assessments of client needs and oversee formulation and development of Person Centered Plans.
- Lead continuous progress review and evaluation; utilize assessment tools; manage referral methods and case consultations; ensure documentation is accurate.
- Assist Support Coordinators in determining clinical interventions and problem solving; supervise administrative duties; provide training on inter- and intra-agency services and clinical interventions.
- Oversee submissions to maintain services under the DD Waivers (Service Modifications, Renewals, etc.) including WaMS; participate in records and utilization reviews.

- Oversee use and maintenance of the electronic health record to document Support Coordination performance and service delivery.
Graduation from an accredited college or university with a bachelor’s degree in psychology, social work, or a related field; plus five years of experience working with individuals with developmental disabilities; or a bachelor’s degree and a valid Qualified Developmental Disabilities Professional (QDDP) certification plus five years of experience. Masters can be substituted for 1 year of experience; or graduation from an accredited college or university with a doctoral degree in psychology, social work, or a related field.
CERTIFICATESAND LICENSES REQUIRED
- Valid motor vehicle driver’s license with fewer than six demerit points.
- CPR certification required within 3 months after appointment and maintained.
- QDDP certification required within 1 month of hire.
- QIDCM certification required within 1 month of hire.
