Our mission-driven and member-focused company is seeking an experienced and self-motivated Community Health Worker/Community Guide who has worked with individuals living with serious mental illness and substance use disorder, and understands the unique needs of this community.

In this role, the Community Guide will be working with the multidisciplinary care team to plan and coordinate services for members.

Responsibilities
  • Build genuine, positive relationships with a diverse range of members and improve health and wellbeing through daily work on an integrated technology platform to call and text members, schedule visits and clinical appointments, and document interactions for the clinical team.
  • Engage in outreach and engagement into care:
    Engage and work alongside clients from diverse ethnic backgrounds, focusing on those with severe and persistent mental illness, substance use disorders and complex medical conditions. Reach out to members to enroll them in our care model.
  • Manage a caseload of 60-100 members and provide coverage for other Community Guides during periods of absence or increased workload.
  • Assess member needs and social determinants of health to help create patient-centered care plans.
  • Support member enrollment in behavioral and mental health services and build trusting relationships with members.
  • Coordinate referrals and support appointment scheduling and attendance with behavioral health, primary care, and other provider types.
  • Organize and prioritize workload with effective time management to achieve monthly goals and outcomes.
  • Maintain confidentiality and professional boundaries, seeking consultation as needed from the multidisciplinary team.
  • Visit members in person as appropriate (home, community, hospital or other treatment settings).
  • Use text, phone, email and in-person interactions to match communication with members’ preferences.
  • Identify and connect members to community and health resources, including transportation when needed.
  • Prepare members for services ending by promoting self-sufficiency and independence.
Documentation
  • Complete accurate documentation in the Reema App, keep a current care plan, and comply with HIPAA.
  • Support member records in Reema’s technology and EHR.
Representation
  • Work independently and collaboratively in a fast-paced startup environment; adapt to frequent change.
  • Serve diverse members with dignity and respect; build relationships with community resources and customers.
  • Collaborate with the multidisciplinary team to share perspectives and learn from others.

Education, Experience and Skills
  • Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences or related fields (experience may substitute for education).
  • Fluent in Spanish
  • 3+ years of direct experience with the population or community served
  • Interest in working with individuals with significant mental and chemical health needs
  • Experience with crisis planning, case management, boundaries and safety in the community
