RN Job Description

Provide skilled, complex nursing care and supervision to clients according to the Plan of Care and Sanzie Healthcare Services Inc.'s standards and policies. Services include:

• Participate in establishing and implementing the client's service plan in coordination with staff.
• Provide services hourly or per visit to meet professional care requirements.
• Assess the nursing needs of the client regularly.
• Conduct initial face-to-face assessments at the client's home.
• Provide nursing services as needed per the client's service plan.
• Work with clients and families to determine needs and access the home environment.
• Report client progress and problems to supervisors, personnel, or physicians.
• Supervise CNAs and other home health aides.
• Perform annual assessments and supervise tasks of PCA, CNA, and other staff.
• Supervise personal care tasks for medically compromised, blind, or sensory-impaired clients.
• Admit and assess clients, complete clinical documentation.
• Supervise supervisory visits and respond to service complaints.
• Provide hands-on care, case management, and education on proper care and treatment.
• Revise care plans in consultation with physicians based on ongoing assessments.
• Direct nursing and home healthcare as prescribed.
• Review treatment plans for accuracy and completeness.
• Assist in orientation and annual evaluations of caregivers.
• Perform supervisory visits to observe and advise caregivers, ensuring client satisfaction.
• Assess and educate about pain management techniques.
• Ensure safe client care environments, report condition changes, and demonstrate medication knowledge.

Salary: $40 - $50 per visit
