Exeter City PCN provides a prescription support service for member practices and we are looking for a dynamic, proactive individual to join our PCN Pharmacy team as a Prescription Co-Ordinator to cover maternity leave. This role would suit someone who already works within a GP practice prescribing team or who has knowledge of issuing prescriptions. You will be working alongside the existing PCN Pharmacy team, comprising of Pharmacy Technicians and Clinical Pharmacists, at the HUB office to deliver an effective Prescription Service to our PCN.

This is a developing role and may become permanent in the near future as the service grows. You will be expected to undertake training, work under the guidance of the PCN pharmacy team, and your role may be amended as the service expands.

Main duties of the job

KEY RESPONSIBILITIES:

You will work in a supportive role within the team working alongside pharmacists, Pharmacy Technicians and member practices of the PCN.

Liaise with patients, healthcare professionals, product suppliers and community pharmacies about prescription requests.

Act as a point of contact for the PCN regarding prescriptions.

Receive multiple medicine requests, prioritise and process in accordance with agreed target times and clinical priority.

Use skills to ensure incoming requests and manage/prioritise these, ensuring they are dealt with in a timely manner and referred to the correct member of the team.

Supporting the team to coordinate the day-to-day duties.

Work to your competency and ensure training is up to date.

Ensure all errors are recorded and reported to your line manager.

Exeter City Primary Care Network (PCN) brings together six local GP practices in the centre of Exeter, collectively supporting around 39,000 patients. The PCN enables partnering practices to work collaboratively at scale and provide their patients with access to additional services. This neighbourhood approach to care is guided by the principle that each practice best understands its patients' needs and will work together to foster collaboration, deliver services, and enhance care for the broader population.

These services include the Same Day Physio and Mental Health Service, Care Home Ward Round Team, Womens Menopause Support Service, Clinical Pharmacy Support, Wellbeing Coaches, Nutritional and Diabetes Workshops, Cancer Care Services, and coordinators supporting patient education, health service navigation, and signposting.

This role will join the pharmacy team providing a full prescription service to all member practices.

Job responsibilities

PROFESSIONAL

Ensure you read and understand all Protocols and SOPs, bringing to the attention of your line manager any gaps in knowledge for training and support where needed.

Attend regular team briefs and participate any ideas, concerns to the team.

Respond to patient - service uses via email - telephone and tasks in a timely manner.

To participate in recording and reporting any near miss, take accountability and ensure learning to prevent further occurrences.

Be responsible for your workload and respond to patients, practices effectively and within competence.

Ensure national guidelines and standards are followed as appropriate.

Advocate good and effective communication both internal and external to the service.

Effectively disseminate and escalates information through the appropriate reporting channels, ensuring timely response and maintained awareness of best practice.

Undertake all duties and make your line manager aware of any duties that cannot be fulfilled.

To identify solutions to improve service delivery and discuss with your line manager.

To be responsible for your own timekeeping.

PEOPLE MANAGEMENT

To directly contact other members of the team/ line manager for any help and support needed.

Ensure training is up to date and you have the required skills to successfully complete day to day tasks. Be responsible for recognising where development is needed.

Be able to work in a friendly and professional manner, whilst dealing with patients, practices and external MDT teams. Work within your scope of competence.
