Would you like to join a friendly, dynamic & supportive service with scope to work within Exeter Community Rehab (ECR) and Urgent Community Response (UCR) based in one hub in Central & East Exeter? We are seeking a Band 3 Community Support Worker to join our team based within the Exeter Community Rehabilitation Team - Central & East.

We aim to provide high quality care to maximise independence and quality of life at home through a strength-based approach. This is completed within a supportive network of experienced therapists offering a structured induction, time to shadow other community teams, peer support groups and opportunities for career development.

The focus of rehabilitation is paramount to patients which we are proud to support through individual patient assessments and treatments, alongside rehabilitation groups for strength and balance. We provide a variety of settings for patient treatments including within their own homes, care homes and gym spaces (NHS and Leisure Centres). In addition, we carry out virtual assessments and treatments facilitating the plan for the NHS digital transformation.

We are also supporting the National Ageing Well Agenda within the NHS Long Term Plan and this provides you with the chance to be at the forefront of change within Exeter.

The UCR function covers 8am-8pm across 7 day working and provides a 2 hour response to admission avoidance patients within the community, as well as continuing with the discharge to assess pathway. This post holder would be required to work on a rota basis within these hours.

Your caseload involves a variety of patients to be seen, including and not exclusively:

  • Working with patients with long-term conditions
  • Frailty
  • Neurological conditions
  • Respiratory conditions
  • Trauma, orthopaedic and musculoskeletal conditions
  • Moving and handling including equipment prescription
  • Patients in Short Term Rehabilitation Beds within Residential and Nursing Homes

Tasks may include but are not exclusively limited to:

  • Assisting therapists with the patient assessment process
  • Carrying out treatment programmes e.g. mobility & transfer practice, drink & meal preparation, cognitive tasks, personal care activities, exercise programs, practicing use of equipment
  • Ordering equipment
  • Carrying out other therapy related administration tasks
