What you will do:

• Conduct Care Act Assessments, Mental Capacity assessments, and facilitate Best Interest outcomes.
• Complete Community Deprivation of Liberty assessments and engage in Continuing Healthcare (CHC) checklists and Decision Support Tool (DST) processes.
• Operate from a Strengths-Based perspective, focusing on Least Restrictive Practice.
• Support the team by triaging referrals and prioritising day-to-day case management activities.
• Collaborate confidently with professional partners within a multidisciplinary team (MDT) approach.
