The post holder will work as a Support Worker to a client within their own home. They will work closely with the registered manager and nominated individual to undertake delegated activities under supervision.
Areas covered - Torquay, Exeter, Newton Abbot, and surrounding areas
- Full-time and part-time available
- Shifts vary from 8am to 8pm
- Male and female positions available
- Travel time and mileage paid at 45p per mile
- Spend a minimum of 3 hours with one client per day
Main Duties:
Maintain good verbal and written communication skills to liaise with clients, families, health, and other professionals, and accurately record information onto MCM.Work as part of a team, providing a coordinated approach to meet the care needs of clients in their own environment.Assess, review care plans, and risk assessments collaboratively with clients, families, and the management team.Communicate with staff, stakeholders, and the public with tact and sensitivity.Assess clients' psychological and physical capabilities, noting hobbies, likes, dislikes, and preferences.Coordinate care and support for clients and contribute to reviews as needed.Demonstrate good IT skills and record information clearly and professionally.Ensure the delivery of a service that promotes choice, empowerment, independence, wellbeing, and dignity.Maintain a flexible approach to work in collaboration with management.Contribute to essential reports when required.Participate in audits to ensure safe and quality care delivery.#J-18808-Ljbffr
