Compensation: $80,000.00 - $90,000.00 / year

Agency-wide leadership for ISP development, assessment-driven goal setting, Therap systems, and outcome monitoring.
This position is central to how the agency plans, delivers, and evaluates services. The Director of Service Planning and Outcomes ensures that every Individualized Service Plan (ISP) is built from meaningful assessment, translated into measurable goals, and tracked consistently to demonstrate progress and inform continuous improvement. This role provides the structure, standards, and oversight that allow programs to deliver high-quality, person-centered services with clarity and accountability.
Key Responsibilities- Provide agency-wide leadership for ISP development, goal creation, and outcome evaluation across all programs
- Chair and steward the Goal Development Committee to ensure high-quality, measurable, and person-centered goals across programs
- Translate assessments and interdisciplinary input into clear, measurable, person-centered goals
- Establish and maintain standards for goal quality, progress measurement, and revisions
- Serve as the agency s lead for Therap – Electronic Health Record system integration and integrity, including naming conventions
- Data accuracy and consistency; ensure consistent use of Therap for goal tracking and outcome reporting based on data
- Monitor goal progress and outcome data to identify trends, gaps, and improvement
- Lead the agency s outcomes reporting framework to support quality improvement and regulatory readiness
- Partner with program leadership, QA, and clinical teams to strengthen service planning practices

- Agency-wide in scope
This role is not:
- Site-based operations management
- Staffing, scheduling, or facility oversight
- Deep experience with ISP development and Therap
- Strong understanding of assessment-driven planning and outcome tracking
- Comfortable setting standards and guiding others without direct operational management
- Enjoys building systems, improving consistency, and using data to drive quality
