Jail Transitions Case Manager

Overview

Summary:

Responsible for facilitating the successful reintegration of individuals exiting the jail system into the community. This role provides support, resources, and guidance to clients, helping them navigate challenges related to housing, employment, mental health, and substance use. The Jail Transition Case Manager will work collaboratively with various stakeholders, including law enforcement, social services, and community organizations to promote a smooth transition and reduce recidivism.

Position: Full-time

Pay: $27.65 - $29.70/ hour
