• Master’s degree in Nursing, Business, Social Work, or related healthcare field
• Active Washington Registered Nurse license
• 6+ years of Case Management experience
• 2+ years of supervisory or management experience, preferably in Case Management

Full-time, day shift Manager Care Management RN overseeing hospital-based case management, care coordination, and discharge planning services in a leading acute care medical center serving diverse patient populations.

The Manager Care Management RN provides daily leadership and oversight of the Case Management Department, including care coordination, discharge planning, utilization management, social work services, and pre-admission case management. In this role, you’ll guide a multidisciplinary team, collaborate closely with physicians and clinical staff, and serve as a liaison between patients, families, community agencies, and post-acute partners. You will establish and monitor best practices and clinical standards to ensure high-quality patient outcomes and responsible resource utilization.
