Roles & Responsibilities Lead discharge planning and coordinate patient transitions to the next level of care with clarity and compassion
Perform care coordination across units and campuses to ensure timely, safe discharges
Conduct utilization review and needs assessments, including DME ordering and plan-of-care development
Document thoroughly in Epic and communicate care plans to interdisciplinary team members
Collaborate with Social Services and clinical teams to resolve barriers and improve patient flow
Float across hospital areas (including Colby and Pacific campuses) to support unit needs and maintain continuity of care
Qualifications Current RN licensure (pending licenses accepted) and active BLS certification
Minimum 1 year acute care experience with discharge planning or care coordination
Solid case management experience — Med Surg, LLOS, and ED backgrounds are strongly preferred
Comfortable with utilization review and CMS-related regulatory considerations
Able to float between hospital areas and work every other weekend as scheduled
Experience charting in Epic preferred
Knowledge & Skills Discharge planning and care coordination expertise
Case management and utilization review proficiency
Acute care clinical judgement across patient populations (neonates through geriatrics)
Familiarity with Epic electronic health record workflows
Strong interdisciplinary communication and patient advocacy skills
Knowledge of CMS regulations and documentation standards
Ability to manage higher patient ratios and complex caseloads
