About the Position
The RN Case Manager is responsible for coordinating and managing patient care across the continuum to ensure quality outcomes, efficient resource utilization, and patient satisfaction. The role involves assessing patient needs, developing care plans, coordinating services, and facilitating communication between patients, families, and healthcare providers.

Requirements

Education:

ADN or BSN in Nursing (BSN preferred).

License:  Active RN license (state-specific).

Certifications:

BLS required; CCM (Certified Case Manager) preferred.

Experience:

Minimum 2 years of acute care nursing experience; prior case management or utilization review experience preferred.

Skills:

Strong clinical assessment and critical thinking skills.

Excellent communication, organization, and problem-solving abilities.

Knowledge of insurance, discharge planning, and community resources.
