Optum is seeking a Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Snohomish County, WA.
Position Highlights- Dedicated clinicians and interdisciplinary teams focus on assisted living and other community‑based settings, blending palliative care with internal medicine.
- Extensive training in a unique approach that prioritizes quality patient outcomes over volume.
- Manage 4–6 patient visits per day, giving you time to invest in patient well‑being and build meaningful relationships.
- Conduct comprehensive assessments.
- Manage acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
- Manage patients during short‑term sub‑acute rehabilitation episodes.
- Ensure complete, accurate documentation (including ICD‑10 codes).
- Communicate and collaborate with the interdisciplinary care team.
- Conduct advanced illness and advanced care planning conversations.
- Provide patients and caregivers with counseling and education.
- Perform comprehensive assessments and document findings in a concise manner compliant with CMS regulations.
- Facilitate agreement and implementation of the patient’s plan of care by engaging facility staff, families, and clinicians.
- Coordinate care as patients transition through different levels and settings.
- Review orders and interventions for appropriateness and response to treatment, identifying the most effective plan of care.
- Function independently and responsibly with minimal supervision.

- For Nurse Practitioners:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Active and unrestricted Nurse Practitioner license in the state of residence (or ability to obtain by start date).
- Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following areas:
Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, Adult‑Gerontology Acute Care Nurse Practitioner, Adult‑Gerontology Primary Care Nurse Practitioner.
