Sign-on Bonus: $10,000 for eligible external hires
Pay Range: $45.67 – $85.73 per hour

Key Responsibilities- Coordinate care and discharge planning for patients in an acute inpatient setting
- Perform comprehensive assessments and develop individualized care plans
- Collaborate with physicians, nurses, and interdisciplinary teams to ensure continuity of care
- Support appropriate resource utilization and advocate for patients and families
- Associate Degree in Nursing required; BSN preferred (or completed within 3 years of hire)
- Current RN license in the state of Washington
- Minimum 2 years of acute care experience in a med-surg or inpatient setting
- Prior experience in care management or utilization review preferred; case management certification a plus
