** Hospice LPN – Weekend On‑Call (Part‑Time, 20 Hours/Week)

** Saturday & Sunday, 8:00 a.m. – 6:00 p.m. (On‑Call)

** Snohomish County, WAThe Hospice Licensed Practical Nurse (LPN) provides compassionate, patient‑centered nursing care to individuals receiving end‑of‑life services in their homes or residential settings. This weekend on‑call role ensures timely response to patient needs, urgent visits, symptom management, and support for families during scheduled hours.##
** Key Responsibilities
*** Provide on‑call coverage during scheduled weekend hours, responding promptly to patient and family needs
* Perform in‑home visits for urgent assessments, symptom management, and changes in condition
* Implement nursing interventions based on the established plan of care and hospice protocols
* Communicate changes in patient status to the RN Case Manager, interdisciplinary team, and on‑call leadership
* Document all care provided in accordance with state, federal, and agency standards
* Provide emotional support and education to patients and families regarding disease progression, comfort measures, and hospice philosophy
* Collaborate with interdisciplinary team members to ensure continuity and quality of care
* Follow infection control practices and maintain safety standards in all care environments
* Participate in on‑call handoff reports to ensure smooth transitions between shifts
* Maintain accurate medication records and assist with medication reconciliation during visits##
** Qualifications
* ** Active Washington State LPN license in good standing
* Minimum 1 year nursing experience; hospice, home health, or acute care preferred
* Valid driver’s license and reliable transportation for home visits throughout Snohomish County
* Current BLS certification
