Position DetailsPosition is PRN (as needed) and requires availability of 3‑6 hours per week, sometime Monday‑Friday between 9:00‑3:30. You will receive your schedule in advance and it is based on RN availability. There is potential to increase hours over time based on census. This position works with the Adult Partial Hospitalization (PHP) and Intensive Outpatient (IOP) population.
Role SummaryThe Registered Nurse will play a crucial role in delivering high‑quality patient care within partial hospitalization and intensive outpatient programs. This position requires a strong commitment to mental health advocacy, patient education, and clinical excellence.
Responsibilities- Assessing and monitoring patients' physical and mental health status, including vital signs, medication management, and symptomatology upon admission and as needed for ongoing client medical needs.
- Collaborating with the interdisciplinary team to develop and implement individualized care plans based on patients' needs and treatment goals.
- Oversight of the medication self‑administration process during program and ensuring all medications that are brought to the clinic by the caregiver/guardian for adolescent clients are logged in and out daily per policy.
- Providing patient education on mental health conditions, treatment options, and self‑care strategies to promote wellness and recovery.
- Conducting therapeutic groups and individual counseling sessions to support patients in developing coping skills and fostering resilience.
- Participating in treatment team meetings, case conferences, and clinical supervision sessions to discuss patient progress and coordinate care.
- Documenting patient assessments, interventions, and outcomes accurately and timely in electronic medical records (EMR) systems.
- Adhering to ethical and legal standards of nursing practice, including maintaining patient confidentiality and respecting autonomy.
- Collaborating with community resources and external healthcare providers to ensure continuity of care and facilitate transitions between levels of care.
- Engaging in ongoing professional development activities to enhance clinical knowledge and skills related to mental health nursing practice.
- Graduation from an accredited school of nursing with a valid Registered Nurse (RN) license.
- Strong assessment, communication, and interpersonal skills to interact effectively with patients, families, and interdisciplinary team members.
- Ability to work collaboratively in a fast‑paced environment and adapt to changing priorities.
- Knowledge of evidence‑based practices in mental health treatment and familiarity with psychiatric medications.
- Current Basic Life Support (BLS) certification required.
- Advanced Cardiac Life Support (ACLS) certification preferred.
- Clinical experience in psychiatric/mental health nursing preferred (PMH‑RN).
