• Perform comprehensive evaluations, assessing the social, emotional, and support needs of patients and families.
• Set measurable objectives formulated in conjunction with the rehab team.
• Assist physicians and other health team members in understanding significant social and emotional factors related to patient health problems.
• Instruct and counsel patients and families in coping with social and emotional responses connected to illnesses.
• Participate in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs.
• Identify and assist patients and/or families in utilizing appropriate community resources to achieve measurable objectives.

• Master’s Degree in Clinical Social Work (LCSW).
• Current licensed clinical social worker licensure in the state of practice.
• Minimum one year of experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury, or acute diagnosis.
• CPR certification required.
• Ability to lift 50 pounds; moving, lifting, or transferring of patients up to 100 pounds following safety procedures.
• Fine motor skills, visual acuity, and ability to walk/stand for extended periods.
• Physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times.
