Clinical Pharmacist
The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients.
The responsibilities of the clinical pharmacist will predominantly fall into the following areas:
- (1) Patient Care - The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team.
- (2) Population Management - The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.

Required Qualifications:- Bachelor's Degree in PHARMACY from an accredited School of Pharmacy. Or
- PHARMACY from an accredited School of Pharmacy.
- Coursework/Training Postgraduate Year One (PGY-1) Pharmacy Residency Or equivalent educ/experience
- Upon hire:
Washington Pharmacist License - Within 2 years of hire:
Certification from Board of Pharmacy Specialties (Facility Managed) - Upon hire:
Board of Pharmacy Preceptor License (Facility Managed) - when eligible and where applicable
