Position: Licensed Practical Nurse (LPN) – Clinic Setting

On-Site | Primary Care Clinic | Frail & Elderly Population

We’re seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join a mission-driven primary care clinic serving a frail and elderly population. In this role, you’ll be a vital part of an interdisciplinary care team, supporting high-quality, patient-centered care in a collaborative clinic environment.

What You’ll Be Doing

Provide delegated nursing services within a clinic setting under appropriate supervision

Collaborate closely with physicians, RNs, and the interdisciplinary care team to support individualized care plans

Administer medications, vaccinations, and treatments according to nursing standards

Perform clinical tasks such as blood draws (within scope of practice) and patient triage

Observe, document, and report changes in patient condition and treatment responses

Educate patients and caregivers on medications, treatments, and care instructions

Assist providers during exams, treatments, and diagnostic procedures

Support daily clinic operations, including patient flow, appointments, and urgent needs

Respond to incoming calls and address or route patient concerns appropriately

Maintain exam rooms, monitor supplies and expiration dates, and support inventory needs

Participate in interdisciplinary meetings, quality improvement initiatives, and ongoing training

Adhere to all safety, infection control, OSHA, and confidentiality standards

What You Must Have

Graduate of an accredited school of nursing

Current LPN/LVN license in good standing (state-specific)

BLS certification (or ability to obtain within 90 days of hire)

Reliable transportation, valid driver’s license, and required auto insurance

Medical clearance and up-to-date immunizations prior to patient contact

Ability to pass pre-employment screening (background check, license verification, drug screen, etc.)

What We’d Love to See

1+ year of experience as an LPN/LVN (strongly preferred)

Experience working with a frail or elderly population

IV certification (preferred; training available if not currently held)

Strong communication, organization, and patient-education skills

Ability to thrive in a fast-paced, interdisciplinary clinic environment
