🥗 What You’ll Be DoingProvide high-quality nutritional care as an active member of an interdisciplinary clinical team
Conduct initial and ongoing comprehensive nutritional assessments in accordance with program and regulatory standards
Develop, implement, and update individualized care plans in collaboration with clinical and social care professionals
Deliver nutrition counseling and education to patients, caregivers, and families to support wellness and chronic disease management
Coordinate and support nutrition-related interventions
, including home-delivered meals and supplements
Monitor changes in weight, hydration, medications, swallowing ability, skin integrity, mood, and overall intake
, and communicate recommendations to the care team
Provide guidance and consultation to team members, contractors, and center staff regarding nutritional needs and related health concerns
Respect and incorporate cultural, ethnic, and personal dietary preferences into care planning
Participate in team meetings, family conferences, quality improvement initiatives, and staff training
Ensure accurate, timely, and compliant documentation reflective of high-quality patient care
Maintain compliance with safety, infection control, and confidentiality standards
Support other team or food service functions as appropriate based on licensure and competencies
✅ What You’ll NeedBachelor’s degree in food, nutrition, or dietetics from an accredited institution
Registered Dietitian credential (passed the CDR exam)
At least 1 year of experience working with the elderly population
(training support available if needed)
Reliable transportation,
valid driver’s license
, and required auto insurance
Medical clearance and up-to-date immunizations prior to patient care
Ability to pass a comprehensive background check
, including license and exclusion verification
Strong computer skills, including Microsoft Outlook, Word, Excel, and Power Point
Excellent written and verbal communication skills
