Job Description
Peace Health is seeking an LPN Clinic - Family Practice for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at Peace Health is $26.70 – $40.06. The hiring rate is dependent on several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc. Hiring Bonus and Relocation Bonus may be available!
Responsible for ensuring delivery of quality nursing care to clinic patients as directed by physicians and other healthcare providers. The Clinic LPN functions as a core member of the interdisciplinary care team and fulfills/shares the roles and responsibilities of rooming nurse, resource nurse, and clinical support nurse in the ambulatory patient care setting.
- Assesses and implements patient care under supervision. Prepares patient for examination.
- Provides direct patient care. Assists provider with examination and treatment of patient as directed.
- Under the oversight of provider, may be responsible for telephone triage using age specific triage protocol books and computerized resources.
- Documents patient care in health record according to department standards.
- Obtains and conveys information to and from patients, pharmacies, and other providers as directed and supervised by a licensed provider. May obtain referral authorizations for treatment as directed by provider.
- Ensures own professional effectiveness through continued education and professional development.
- Performs other duties as assigned.
Education
- Accredited State Board of Nursing LPN Program Required.
Experience
- Minimum of 1 year Preferred: clinical experience as an LPN.
- Required:
Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient’s needs.
Credentials
- Required:
Upon Hire Licensed Practical Nurse - Oregon. - Required:
Upon Hire Basic Life Support.
Skills
- Basic computer skills required including familiarity with MS Office applications.
- Must have cooperative working and interpersonal skills and the ability to work effectively with a wide variety of patients, families and staff.
- Effective prioritization skills with the ability to manage multiple clinical tasks at one time.
- Must be well organized, detail oriented, flexible and able to function independently.
Oregon:
- LPNs working in the state of Oregon will ONLY perform triage components that align with applicable state regulations within the LPN scope of practice as defined by the Oregon State Board of Nursing.
Lifting
- Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
- Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
- Push/Pull:
Frequently up to 45 lbs. force (i.e., WOW, medical carts). - Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.
- Ability to move around area with frequent sitting.
- Bending/ stooping/ squatting/ reaching/ kneeling frequently.
- Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.
Environmental Conditions
- Exposure to biohazard, body fluids and airborne particles.
- Must be able to complete tasks in a noisy environment.
Mental/Visual
- Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
- Ability to communicate and exchange accurate information.
