Rounding Wound Care Nurse Practitioner

Overview

We are a growing wound care-focused organization seeking a full-time, energetic Nurse Practitioner in Portland, OR, for a Rounding Wound Care Nurse Practitioner role.

• Manage a full patient load across Skilled Nursing Facilities, assisted living, and residential settings, providing direct patient care and implementing treatment protocols.
• Conduct rounds, coordinate care, and communicate effectively with patients, families, and healthcare partners.
• Accurately document all care, complete charting, and ensure compliance with protocols and quality standards.
• Travel in personal vehicle to patient sites; mileage reimbursed.

• Board certification as a Nurse Practitioner, wound care experience, and strong communication skills.
• Preferably certified or pursuing wound care certification.
• Maintain certifications, credentials, and complete ongoing education; obtain wound care certification within one year if not already certified.
