The Opportunity :Our client, one of the largest and most respected home health providers in the western U.S., is seeking a licensed Physical Therapist to deliver in-home rehabilitation services across Eugene and Salem, OR. This role combines clinical autonomy, patient-centered care, and flexible scheduling with the stability of a full-time position and full benefits package.
Key Responsibilities :- Provide physical therapy services in patients homes based on physician-approved care plans
- Assess patient function, mobility, and pain levels to adjust treatment plans as needed
- Use therapeutic exercises, gait training, and mobility techniques to enhance recovery
- Educate patients and caregivers on safe practices and at-home therapy techniques
Maintain accurate, compliant clinical documentation
Collaborate with nurses, care coordinators, and physicians for integrated care delivery
Qualifications (Must-Have) :- Masters or Doctorate in Physical Therapy
- Active Physical Therapist License in Oregon
- Current CPR / BLS certification
- Minimum of 1 year of experience as a PT (Home health preferred)
Valid drivers license and insured vehicle
Willingness to travel throughout Eugene, Salem, and nearby areas

Strong clinical judgment and communication skills
Experience working independently while collaborating with multidisciplinary care teams
