MDS Coordinator RN or LPN

As an MDS Coordinator, you will work collaboratively with an interdisciplinary team to assess residents’ needs and optimize their care plans. Position is onsite.

• Facilitate the pre‑admission process, securing essential information for MDS optimization
• Review resident care and set ARDs to ensure accurate Prospective Payment System MDS Assessments
• Maintain state and federal compliance with timely and precise resident assessments
• Accurately code MDS data using information from staff interviews, medical records, and resident observations
• Oversee MDS schedules and assist with other assigned responsibilities

• Active RN License in Oregon (or ability to obtain)
• Strong interpersonal communication and teamwork skills
• Experience with the RAI process and Case Management
• Willingness to travel frequently across Oregon
