* THE ROLE:

** Case Managers/Skills Trainers work in partnership with the multi-disciplinary treatment team to support and help manage patients with chronic mental health conditions and substance use disorders. Case Managers/Skills Trainers help stabilize and/or improve social and behavioral functioning in patients, as well as help ensure smooth transitions throughout the continuum of care and reintegration into the community. Case Managers/Skills Trainers also help patients connect with community partners and resources, develop strong personal support systems, as well as teach patients healthy skills that promote the recovery process.

Treatment often focuses on the development of problem-solving/coping skills and learning to utilize appropriate conflict resolution strategies.

Job Requirements
*** Bachelor’s degree; at least 1 year of experience directly related to the duties and responsibilities specified.
* Must be certified as a CDCA (Chemical Dependency Counselor Assistant) in the State of Ohio
