Practice NurseNurse Practitioner

Hassengate Medical Centrehas a fantastic opportunity fora practice nurse/nurse practitioner to join our team on a part-time basis working 30 hours per week signaling.

• Providing consultations in the surgery.
• Participation in the implementation of health promotion disease management and the provision of a full range of nursing services to the patient population of the practice.
• Working collaboratively with other healthcare professionals.
• Following the National Service Framework protocols / NICE guidelines screening patients for disease risks Martech early signs of illness.
• Providing appropriate care to patients.
• Assisting in the assurance that all policies and protocols are regularly re-assessed and reviewed allowing effectiveness.
• Identifying care needs of the practice population and organising work to best meet the need.
• Moving on to other areas as competence and practice needs require such as: family planning and emergency contraception but includes those under 16yrs; menopause advice; chronic disease management within guidelines and competency for diabetes (6 month checks) RA hypertension and CHD with support of GP in need.
