Direct Support Professional - Community Support Facilitator

Location: Carlsbad/Oceanside to Escondido (including Vista and San Marcos)

Schedule: Monday through Friday 8am to 6pm; 3–6 hours daily (15–20 hours weekly depending on client availability)

Salary: $19.50 per hour

TMI’s Primary Purpose – TMI is dedicated to assisting children and adults with intellectual/developmental disabilities such as autism spectrum disorder, Down syndrome, epilepsy, cerebral palsy, and Deaf/Hard of Hearing, to live, work, and participate in their community. Employees work effectively with clients, are sensitive to their needs and preferences, and represent TMI’s vision, mission, core values.

• Assist clients living independently in the community with budgeting, accessing community resources, household management, medical appointments, etc.
• Assist with development and implementation of annual Personal Support Plan (PSP) and/or Supported Living Plan (SLP) goals and objectives.
• Participate in developing a circle of support, both paid and unpaid, to build relationships with family, friends, neighbors, and other community members.
• Develop individualized adaptive support materials as needed.
• Coordinate and monitor other services such as MTS, Medi‑Cal, Social Security, etc.
• Develop and expand clients’ participation in integrated community activities.

• Complete required documentation including progress notes and medical appointment tracking according to established deadlines.
• Attend mandatory meetings and trainings (staff meetings, team meetings, CPR, First Aid, AED certification, Relias web‑based learning).
• Enter timesheets, mileage reimbursements, and cell phone reimbursements into the time‑keeping system by deadlines.
• Maintain communication with supervisor and members of the clients’ planning team.
• Notify supervisor immediately of changes to client schedule and health & safety incidents.
• Respond to supervisor’s calls/texts/emails within 24 hours; urgent calls respond immediately.
• Respond to calls/texts/emails from members of the clients’ planning team.
• Drive personal vehicle (or use public transportation/paratransit) to assist clients; provide proof of valid driver license, annual inspection, insurance, registration as required.
• Comply with federal, state, local reporting & disclosure requirements; HIPAA laws and TMI policies regarding confidentiality of information.
• Other assignments and duties as deemed appropriate by supervisor / program director.

• Education: High School Diploma/GED
• Experience: 1 year in social services, education, or with developmentally disabled population (preferred)
• Driving: 3 years driving experience
