Pay range for this position is $36.68 - $54.40/hour. Actual pay depends on years of licensure. This is an hourly position.

Position SummaryThe RN Case Manager - Home Health is responsible for assessing, planning, implementing, monitoring, and evaluating clinical outcomes, service utilization, and resource management for patients on their caseload. The role encompasses patients of all ages, from infancy to geriatrics, requiring appropriate patient care knowledge and skills. The Case Manager collaborates with providers to coordinate interdisciplinary services including nursing, therapy, social work, home care aides, respiratory therapy, home medical equipment, and home infusion services.
They are accountable for achieving optimal patient outcomes, especially for high-risk patients, providing evidence-based, patient-centered care at the patient's residence, and understanding OASIS documentation and care planning's impact on agency performance.
Required Qualifications:
- Completion of nursing program
- At least two years of RN experience in a healthcare environment
- RN License in IL or MI, depending on the work location
- Current American Heart Association Healthcare Provider BLS certification prior to employment start date
- Valid State Driver's License with proof of active auto insurance
- Excellent interpersonal and communication skills
- Proficiency with Microsoft Office and solid computer skills
- Strong analytical and problem-solving skills with attention to detail
- Own transportation, auto insurance, and adherence to safe driving laws and recommendations
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN)
- Experience in Home Health, OASIS, and Case Management
- OASIS Certification
- Basic proficiency with Electronic Medical Records and Microsoft applications
