Overview
An opportunity has arisen at Epsom & St. Helier University Hospitals NHS Trust to join our Macmillan Cancer Support Worker Team (CSW). The successful applicant will work alongside an established team of Clinical Nurse Specialists, Advanced Nurse Practitioners and medical colleagues supporting patients being investigated and treated for cancer.
Key responsibilities- Support people to navigate the complex health and social care system during and following completion of cancer treatment.
- Coordinate care by providing a single point of access, including rapid re-entry into the system for those identified as having urgent or specialist needs.
- Coordinate care, education and support for people with non‑complex care needs.
- Co‑ordinate education and support for people with non‑complex care needs.
- Implement the National Cancer Survivorship Initiative risk stratification model for level one (supported self‑management) patients.
- Provide information and support to enable cancer patients and their carers to navigate the health and social care system.
Epsom and St Helier University Hospitals NHS Trust provides a range of medical services to approximately 490,000 people living across southwest London and northeast Surrey. In addition, we provide more specialist services, in particular renal and neonatal intensive care, to a wider area, covering parts of Sussex and Hampshire. We serve an area that is rich in diversity, with a mix of urban and rural areas, and differing levels of quality of life.
