Hospital at Home is an exciting service that delivers acute-level care in the comfort of a patient's own home. Working as part of a multi-disciplinary team, you will play a vital role in reducing unnecessary hospital admissions, supporting early discharges, and enhancing patient outcomes through personalised care.

Based across West Essex, including Harlow, Epping, and Uttlesford, you'll work in a fast-paced, collaborative environment that puts patients and families at the heart of everything we do.

Main Duties and Responsibilities

• Provide acute-level care in patients' homes
  , delivering safe, effective, and evidence-based clinical interventions in line with individual care plans.
• Assess, plan, implement, and evaluate care for patients with a range of complex medical conditions who would otherwise require hospital admission.
