Occupational Therapist ($5,000 Incentive) – Legend Oaks – Ennis, TX

Position: Full‑time Occupational Therapist
Location: Ennis, TX
Sign‑On Bonus: $5,000

Legend Oaks is seeking a dedicated Full‑time Occupational Therapist ready to make a meaningful impact within a dynamic, supportive environment. Join our outstanding in‑house therapy team and contribute to facility‑level decision‑making while receiving excellent mentorship and growth opportunities.

Key Responsibilities
  • Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care within regulatory and clinical standards.
  • Utilize standardized assessments and evidence‑based practice to support clinical interventions.
  • Provide rehabilitative, skilled and medically necessary treatment interventions for patients per regulatory requirements.
  • Administer procedures such as ADL retraining, therapeutic activities, therapeutic exercises, modalities, and supportive devices.
  • Assess patient progress and adjust treatments accordingly in the clinical record.
  • Evaluate treatment outcomes for effectiveness, modifying services and making referrals as needed.
  • Consult and counsel patients, families, caregivers, and other service providers.
  • Communicate effectively with supervisors and interdisciplinary team members about patient progress, barriers, and treatment plans.
  • Provide comprehensive discharge summaries per regulatory and clinical requirements.
Qualifications

Licensed Occupational Therapist.
