Jet Health, Inc / Klarus Home Health is seeking compassionate and dedicated Registered Nurses (RN) Case Managers to join our team. We are looking for full-time, part-time, PRN / Per Diem RNs who are passionate about providing quality home health care to our community-based patients in Aurora, Centennial, Glendale, Englewood, and the surrounding areas.

Requirements

• Current State or Compact Registered Nurse (RN) License
• A minimum of one-year clinical experience is preferred
• OASIS knowledge highly preferred
• Demonstrated knowledge and skill in current nursing practice
• Valid state driver's license and automobile liability insurance
• HCHB EMR knowledge is desired
• Follow a designated care plan according to patient's needs
• Make home visits to clients in designated geographic areas
• Perform duties including medication administration, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager
• Follow up on, execute, and document doctors' orders properly
• Perform client assessments
• Manage case coordination
• Accurately document observations, interventions, and evaluations related to client care using a state-of-the-art touchpad tablet

Salary Range: $30 - $50 per hour
