Licensed Practical Nurse

The Licensed Practical Nurse for SoNE HEALTH Medical Group will provide direct patient care and support for the team. This role will function as a clinical lead and point of contact for medical assistant and other staff. Responsibilities for this role will encompass a wide range of clinical tasks, including patient assessments, vaccinations, EKG and Spirometry, and lab coordination. The LPN will need to obtain and maintain Stericycle DOT certification as well as State of CT Backup Vaccine Coordinator.

This position requires strong computer skills, attention to detail, a positive professional attitude, and the ability to work in a team environment.

This position is in Enfield, CT.

• Monitor nurse line & respond to clinical calls.
• Obtain medication and radiology authorizations.
• Complete physical forms, school medication forms, disability paperwork, & all other clinical documentation & forms as needed for patients.
• Review upcoming appointments to anticipate needs, supplies, & completeness.
• Order labs for patients with upcoming appointments and contact patients, as necessary.
• Monitor incoming patient portal messages & return patient calls, as delegated by provider.
• Monitor HIPAA compliant patient text line & respond to clinical inquiries.
• Follow up with referrals to ensure patient has been contacted.
• Prepare & process order forms for patient nebulizers monthly.
• Prepare labs/cultures/specimens to be sent out for testing i.e., confirm labels on specimen, follow office & lab protocols & procedures for transport & processing, ensure labs are placed in outside bin for pick-up on time.
• Prepare & give vaccinations where needed.
• Maintain eyewash station per policies & protocol including testing, cleaning, & reporting.
• Cover clinical staff vacations, sick time, or other time off to include patient care tasks: ie. rooming, triage, vitals, POCT testing, assist provider with in-office procedures, etc.
• Other duties/small projects as delegated by the Office Manager or Provider, within the employee’s legal & personal capabilities.

• Performs other duties as assigned.

• Active LPN License in CT
• Minimum of 2 years primary care office experience required
