Care Coordinator – Integrated Learning Disabilities Service

Initially 3 months, Full time 35 hours per week
Salary: £20 UMB per hour ( £16 PAYE)

Working with Enfield Council, We are looking for a committed and compassionate Care Coordinator to join their Integrated Learning Disabilities Service. This is a rewarding opportunity to support adults with learning disabilities and their carers by delivering high-quality assessments, care management, and coordinated support that promotes independence, wellbeing, and person-centred outcomes.
You will be part of a supportive multidisciplinary environment, working alongside social workers, health professionals, and partner agencies to ensure people receive the right support at the right time.
About the Role As a Care Coordinator, you will:

Undertake initial needs assessments within your specialist area
Develop, manage, monitor and review care and support plans
Work closely with service users, carers, and partner agencies
Coordinate services and placements using available community and independent resources
Ensure care packages remain appropriate, effective, and value for money
Maintain accurate and timely records using council IT and case management systems
Work within financial regulations, safeguarding duties, and council policies
Participate in supervision and performance reviews with your line manager

What We’re Looking For:

We’re seeking someone who can demonstrate:

Experience or strong ability in assessment and care planning
Understanding of the care management process
Ability to work in partnership with service users, carers, and professionals
Knowledge of safeguarding processes and responsibilities
Confidence working within financial and policy frameworks
Strong written and verbal communication skills
Professional, respectful, and person-centred approach
Ability to take responsibility, collaborate, and problem-solve

Desirable:

Experience in integrated or multidisciplinary teams
Experience supporting people with learning disabilities and/or mental health needs
Ability to use alternative communication approaches
