Role Description

This is a full‑time on‑site role for a Social Services Assistant located in Encinitas, CA. The Social Services Assistant will support residents and their families by coordinating social service resources and providing emotional and social support. Responsibilities include conducting assessments, facilitating care plans, and liaising between patients, families, and medical staff to ensure appropriate care. The candidate will also assist with promoting resident well‑being and maintaining records in compliance with facility policies and regulations.

Qualifications
  • Experience in Social Work and Social Services, with a knowledge of best practices in patient and family support
  • Strong Interpersonal and Communication Skills, with the ability to effectively connect with diverse individuals and families
  • Active Listening skills to understand and address resident needs and concerns
  • Attention to detail, organizational skills, and the ability to maintain accurate records
  • Familiarity with healthcare and long-term care environments is a plus
