A Brief Overview
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans.
What You Will Do- Provides psychosocial assessment / intervention to assist patients/families. Provides age specific care to patients/families in all age groups.
- Develops and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs. Develops plans with attention to individual patient and family goals. Provides linkage to community resources to facilitate optimal care.
- Communicates/collaborates with the work team using a multidisciplinary approach
- Performs other duties as assigned.
- Complies with all policies and standards.

- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
- Master's Degree from an accredited school of Social Work (Required)
- Hospital/Health Agency experience. (Preferred)
- Theoretical and clinical knowledge regarding health care delivery (Required proficiency)
- Multi-tasks and prioritizes work (Required proficiency)
- Communicates effectively with persons of various backgrounds (Preferred proficiency)
- Licensed Social Worker (LSW) (Required)
At least 1 year of recent experience is strongly preferred. Previous work in psychiatry or mental health is also strongly preferred. This is a locked, in-patient psychiatric unit. Responsibilities include performing assessments, counseling with the patients and families, providing referrals and community resources, discharge planning, scheduling and planning outpatient follow-up. This position will cover for PTO’s and government requirements. It is needed for rotating weekends and some weekdays as needed.
