- Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team.
- Ensures the development of a safe and effective plan of care through early identification and assessment of the patient's needs and available resources.
- Collaborates by working with patients, families, caregivers, and the health care team locally and within the SHC system to jointly communicate, problem‑solve, and share accountability for optimal outcomes.
- Arranges services among community agencies, physicians, patients, family, caregivers, and other involved in the plan of care.
- Ensures prudent utilization of all resources by evaluating available resources and balancing cost and quality to ensure optimal clinical and financial outcomes.
- Is accountable for achieving optimal outcomes within the standards of practice and contributes to decision‑making and decision support as a member of the interdisciplinary team by ensuring timely sequencing of care and services throughout patient encounters.
- Documents assessments, interventions, plans, and evaluations in accordance with regulatory agency requirements and SHC policy.
- Other duties as assigned.
- Education: BSN required, MSN preferred.
- Certification and Licensure: RN license in state of practice required. Case Management Certification (CMC, ACM, or ANCC) required, or willingness to achieve within 12 months.
- Experience: 3 years of nursing experience required. 5+ years of clinical experience in Pediatrics, Burn Care, and/or Orthopedics preferred. 1 year of care coordination/case management experience preferred.
- Experience with utilization review/management with a working knowledge of Inter Qual criteria is strongly preferred.
