SCI Summary: SCI supports people with disabilities, behavioral challenges, medically complex needs, transitioning youth, the elderly, and other populations using Maryland Department of Health’s Targeted Case Management and Supports Planning work models.

Position Summary: Provide Case Management services to individuals with intellectual and/or developmental disabilities. Establish positive working relationships, explore options and growth opportunities, and maintain a Person-Centered Plan (PCP) with ongoing support services. Ensure all duties meet state-mandated requirements and continuously monitor service quality.

SCI Team Member Expectations: All SCI Team Members follow the organization’s mission, core values, policies and procedures; maintain HIPAA confidentiality; collaborate across teams; participate in meetings and trainings; comply with employment laws; practice self-direction and person-centered planning; foster diversity.

Responsibilities
  • Advocate on behalf of individuals, provide resource information, and educate them to become more self-determined.
  • Collect information for DDA eligibility and recommend eligibility and priority; meet with individuals/families; complete assessments; plan and update PCPs.
  • Plan, facilitate, develop, and update PCPs documenting service needs for DDA-eligible individuals.
  • Research, identify, refer, and coordinate resources to achieve PCP outcomes; share resources with team; assist transitions.
  • Monitor PCPs and conduct follow-up activities; conduct on-site visits and timely contacts; submit required documents (RECON, AIR) and update programs as needed.
  • Adhere to SCI policies and procedures; maintain HIPAA records and use tracking tools; collaborate and communicate effectively; uphold accountability and diversity values.

    Education Requirements
    • A bachelor’s degree in a human services field; OR
    • A bachelor’s degree in a non-human services field with two years of human services experience
    Experience Preferred

    Experience working with people with developmental disabilities
