RN - Home Health

Days, 5x8s;
Monday to Friday, 8 AM to 5 PM, with on-call responsibilities (approximately 1-2 weekdays a week and 1 weekend per month).

The Home Health Registered Nurse (RN) is responsible for providing quality nursing care to patients in their homes. The RN will be part of an on-call rotation, answering patient inquiries and conducting after-hours visits as needed. The RN will work collaboratively within an interdisciplinary team to ensure comprehensive patient care delivery.

• Provide nursing assessments and care for patients in a home health setting.
• Participate in on-call rotation duties, answering calls and addressing patient needs.
• Document patient care and progress in accordance with policies and regulations.
• Coordinate with multidisciplinary teams to develop and implement patient care plans.

• Minimum of 1 year of nursing experience required; home health experience preferred.
• Active state RN license or compact license required.
• BLS certification required.
