Position: Public Health Specialist-Strategy & Planning (HHS)

Overview

This position is responsible for assessing and strengthening policies, systems, and environments that improve health for all in Walworth County. The role serves as a health resource specializing in community health assessment and improvement, health strategy, equity, and partnership development. It facilitates continuous quality improvement for the division through performance management and workplan assessment and monitoring. The position establishes community partnerships to develop or implement programs, initiatives, and policy, systems, and environmental (PSE) changes in alignment with agency and community strategies.

Responsibilities
  • Lead community health planning initiatives, including the Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). Recommend and support data-informed strategies and programs aligned with evidence-based practices and the CHIP or other community initiatives.
  • Identify and implement tasks to facilitate public health’s presence as the Chief Community Health Strategist by developing the division’s health in all policies framework. Recommend policy, systems, and environmental (PSE) changes to improve population health.
  • Work with community leaders and members as well as residents, funders, employers, businesses, appointed and elected local officials, and others to improve resident health, as measured by benchmarks from health planning initiatives and other state/local plans and data sources. Collaborate with the leadership team, staff, community partners, and government officials.
  • Facilitate implementation of health impact assessments to promote community health improvement in diverse partnership settings, providing insight on physical, environmental, and social health and wellness conditions as related to policy, systems, and environment.
  • Guide continuous quality improvement based on best-practice models, performance management assessment, and public health plans. Make recommendations and implement new practices for public health programs; revise existing programs and policies based on best practices and evidence-based programs.
  • Act as a liaison and representative on community committees and coalitions, share resources, and encourage collaboration to meet common goals that support the Department’s values, the Community Health Improvement Plan, and the Wisconsin Health Improvement Plan.
  • Serve as a resource and advocate for health and health literacy in the county and promote equity, quality, and accessibility for all, including vulnerable populations.
  • Respond to inquiries from the general public and provide technical assistance and consultation to community agencies regarding public health programs and health in all policies.
  • Participate in department and community emergency response training, response, and drills in support of public health emergency and disaster preparedness as appropriate to the job.
  • Contribute to a work environment where continuous quality improvements in service and professional practice are pursued.
  • Adhere to ethical and legal principles and department policy in the collection, maintenance, use, and dissemination of data and information.
  • Demonstrate a commitment to county safety and risk management efforts.

Education And/or Experience
  • Bachelor’s degree in public health, health promotion, communication, education, administration or related field with a minimum of six months related experience. Experience working with local public health agencies preferred. Internships may qualify. Knowledge of Trauma Informed Care Principles is essential. Community Health Education Specialist certification preferred but not required.
